Abstract

CV. Surya Mega Yokasa is an industrial company that producing shoes. In the production process, product failures or defects often occur. This reducing the level of productivity of the company resulting in decreased product quality. Analysis of the phenomenon is needed. Analysis of the causes of product failure is using the Failure Mode and Effect Analysis (FMEA) method. Find the highest risk level of product failure using Grey Relational Analysis (GRA) method. Find out the product quality improvement strategy using Root Cause Analysis (RCA) method. The results of this study that the main cause of product failure is a loose sole and torn upper. Each factor has a Grey relationship degree value of 0.33 and 0.39. Proposed improvements to increase the quality of shoes is increasing the skills of workers, doing maintenance of machines, ensuring the quality of raw materials, and improving the quality control system in production process.
