Milkfish, a highly nutritious freshwater fish, is processed by Bunda Foods into various products, from fresh to frozen fish. By 2023, defects have increased from 15% to 23% of total production of 100 pcs. The study aims to identify the most common defects in processed milkfish production and determine the highest Risk Priority Number (RPN) value. The Seven Tools method and Failure Mode Effect Analysis (FMEA) are used in this research. The findings suggest that the most common defect was unfresh fish (143 occurrences) and the highest RPN value was for off-taste (224 occurrences). Both defects were attributed to worker error, highlighting the need for supervision, standard operating procedures, and additional training.
