This study aim to determine and test how much democratic leadership style (X1), information system (X2), and kaizen 5S (X3) on the performance of employees at Alfamidi branch Sidoarjo (Y). Collecting data by questionnaire with simple random sampling as many 167 as minimal respondent. This research method uses multiple linear regression wuth SPSS 22 data processing. The results show that democratic leadership style (X1) partially has positive and significant effect on the performance of employess at Alfamidi branch Sidoarjo (Y), then informastion system (X2) partially has positive and significant effect on the performance of employess at Alfamidi branch Sidoarjo (Y), while kaize 5S (X3) partially has no effect on the performance of employess at Alfamidi branch Sidoarjo (Y). Simultaneously show democratic leadership style (X1), information system (X2), and kaizen 5S (X3) has positive and significant effects on the performance of employees at Alfamidi branch Sidoarjo (Y).
