Abstract

PT. Karya Mitra Teknik is a company engaged in heavy equipment rental services. Based on the results of observations and interviews, there were several problems in forklift maintenance, which should have been avoided or even did not occur, most of these problems occurred due to undisciplined forklift operators and overuse of forklifts. This study aims to determine the cause of the damage and provide suggestions for improvement. Data processing uses Failure Mode And Effect Analysis (FMEA) and Fault Tree Analysis (FTA). Results of data processing using FMEA will be thoroughly analyzed using the FTA method. The results of the study showed three failure modes that required priority repair, namely, radiator failure mode was damaged with an RPN value of 126, starter dynamo failure mode was damaged with an RPN value of 120 and oil failure mode did not flow to the engine with an RPN value of 105.
