The xyz laboratory has a high potential for occupational hazards due to the use of hazardous chemicals and physical activity in the process of making cylindrical concrete test specimens. Lack of risk control causes a high frequency of accidents, such as respiratory disorders, muscle injuries, and even lacerations. This study aims to determine the level of risk and formulate effective corrective measures in controlling the risk of occupational accidents in the xyz laboratory. The methods used are Hazard Identification, Risk Assessment, and Risk Control (HIRARC) and Fault Tree Analysis (FTA). Data were collected through observation and interviews with laboratory workers, then analyzed using a risk matrix and fault tree diagram. The results of the study indicate 16 occupational accident risks, consisting of 13 low-category risks, 1 medium-risk risk, and 2 high-risk risks. High risks include respiratory disorders when weighing cement and the concrete capping process.
