This research is motivated by increasing work accidents at PT XYZ's transit warehouse, which has seen a sharp increase from 4 incidents in 2019 to 13 incidents in 2022. The dominant incidents have been fires or electric shocks, being hit by forklifts, and hot splashes from melted plastic bottles. To overcome this problem, a combined approach of HIRARC (Hazard Identification, Risk Assessment, and Risk Control) and Fault Tree Analysis (FTA) is employed to identify potential hazards, assess the level of risk, and formulate effective control measures. The identification results revealed one high level risk (fire/electric shock) and two moderate levels (being hit by a forklift and splashing heat). At the same time, the FTA analysis showed that the failure came from a combination of human factors (unalert operator, unaware workers of danger), equipment (open wires, unmaintained forklift, inaccurate temperature sensor), and environment (humid area, no zoning, high charging temperature).
