This study analyzes the implementation of the Elderly Posyandu Program at Posyandu Salak, Canggu Village, Mojokerto, using a descriptive qualitative approach with interviews, observations, and documentation. The analysis is guided by four variables of policy implementation: communication, resources, disposition, and bureaucratic structure. Findings reveal that the program has not been fully optimal. Information dissemination, mainly through WhatsApp groups and verbal announcements, is clear but limited in timing and coverage. In terms of resources, the number of cadres and facilities is sufficient, yet training is irregular and socialization to the community remains minimal. Cadres demonstrate enthusiasm and commitment despite receiving relatively small incentives. Activities follow the five-table SOP with functionally organized task distribution. However, improvements are needed in communication strategies and consistent training to ensure broader participation and greater effectiveness. Strengthening these aspects is crucial for achieving equitable and sustainable outcomes in elderly health and welfare.
