Speech delay is a common developmental disorder in toddlers, potentially affecting future social and academic abilities. This study aimed to identify internal and external factors associated with speech delay in toddlers aged 2–5 years at RSI Siti Hajar Sidoarjo. Using a descriptive analytic design with a cross-sectional approach, 156 toddlers were selected through purposive sampling. Data were obtained via questionnaires and medical records. Results showed 72 toddlers (46.2%) experienced speech delay. Internal factors included age >3 years, male gender, premature birth, and low birth weight. External factors were care by non-nuclear family, low parental education, gadget use >3 hours/day, bilingual environment, working mothers, and ≥3 children in the family. Both biological and environmental factors contribute to speech delay. Early intervention and parental awareness are key to preventing long-term impacts.
