This study analyzes the maintenance of CNC milling machines at PT. IJA Surabaya using the Failure Mode and Effects Analysis (FMEA) methodology. The approach involves identifying potential failure modes, assessing their effects and causes, and calculating the Risk Priority Number (RPN) for various components. Data collection includes evaluating severity (S), occurrence (O), and detection (D) scores on a scale of 1 to 10, which are then used to derive the RPN. The analysis reveals that the spindle component is the most critical, with failure modes of wear or seizing impacting production accuracy. The primary causes identified are inadequate lubrication and overloading, resulting in an RPN of 933, categorized as very high risk. The air supply and drill bit components also have the highest RPN and need require better maintenance. Recommended preventive measures include establishing a routine lubrication schedule and implementing load monitoring protocols.
