Abstract

This study was conducted at a cardboard printing company in Sidoarjo to address the increase in defective products that exceeded the 1% quality threshold over the past three months, reaching 1.5% in September and 1.6% in both October and November. The method used was Six Sigma with the DMAIC (Define, Measure, Analyze, Improve, Control) approach. Data was collected through interviews and observations of common defects such as inaccurate cuts, tears, ink smudging, and ink depletion. The average DPMO value reached 3,874.569 with a sigma level of 4.16. Root cause analysis using a fishbone diagram identified five main contributing factors: man, machine, method, material, and measurement. Improvement strategies were developed through SWOT analysis and included technical training, implementation of a real-time monitoring system, SOP standardization, and regular quality audits. The control phase was carried out through routine evaluations, operator certification, and a production documentation system.
