The problem faced by this company is the high number of defective products such as dirty covers on epassport cover products that occurred from July to December 2024. The purpose of this study is to determine the average value of DPMO and the largest type of reject based on the highest RPN value, then provide recommendations for improvement of product quality. The methods used in this research are qualitative and quantitative methods with six sigma and FMEA approaches. The results of this study show an average DPMO value of 1155.47, an average sigma value of 4.55 with the highest defect percentage value of 66.92% in the dirty cover. Improvements can be made through machine maintenance according to SOP, periodic checks, and thorough supervision of every aspect that has the potential to cause failure.
