PT. XYZ faces challenges in liquid glucose production due to decreased machine performance, frequent maintenance, and factors related to raw materials, employees, and consumers. Production capacity drops by 20% of the daily target, reducing overall effectiveness. This study aims to identify the causes of production delays, evaluate failure risks, and develop solutions to enhance efficiency. The methods used include the Fishbone Diagram to identify root causes, FMEA to assess potential failures, and AHP to analyze and prioritize solutions. The results indicate that the main causes of delays are inadequate machine maintenance and external constraints such as raw materials, employees, and consumers. Recommendations derived from AHP and FMEA assist the company in optimizing machine maintenance and improving overall production management.
