This research analyzes the productivity of service and manufacturing companies operating in the fields of consulting, design, fabrication and installation. In the period September 2023 to August 2024, the company was only able to complete 78% of orders, which resulted in customer dissatisfaction and a decrease in the number of orders. This research aims to calculate total productivity factors using the Objective Matrix (OMAX) and analyzing the factors causing the decline in productivity using Fault Tree Analysis (FTA). The OMAX calculation results show a total productivity of 47 with an index of 25.79%, where the number of machines is the main factor influencing operational performance. Meanwhile, FTA analysis revealed various root causes, including unavailable spare parts, mismatched maintenance schedules, untrained operators, as well as external factors such as sudden order requests, changes in production schedules, high machine costs, limited suppliers, and long delivery times.
