PT. Amanah Sang Surya is a company engaged in the production of bottled drinking water under the Suli 5 brand. This research aims to identify the types of defects that most frequently occur and the factors that cause them, as well as looking for effective alternative suggestions for improvement using the Six Sigma application method to measure the level of defects in the production process. Recommendations for improvement use the Failure Mode and Effect Analysis (FMEA) and 5W+1H methods to repair the most dominant defects. The research results show that the quality control of AMDK products is in a stable and statistically controlled condition as seen from the p control graph with a number of defects of 4641.24 DPMO with an average sigma value of 4.10. The factor causing product defects is the machine. The results of calculations using the FMEA method produce the highest RPN value, namely 192.
