Abstract

PT ABC is a shoe menufacturing company. In the production process, the company often faces the problem of product defects, which reach an average of 0,9% per month from 55.504 pairs of shoes produced from January to May 2024. This defect causes losses to the company, both in terms of repair costs, replacements, and decreased revenue. This study aims to identify the types of failures and root causes of defects in shoe products and formulate defect control strategies. Using the House of Risk (HOR) method equipped with pareto diagrams and Fault Tree Analysis (FTA). From the calculation results, the highest ARP value is 819 from the risk event in the oblique stitching process. The mitigation strategy given to reduce the level of disability is to provide training for workers, conduct more monitoring, and implement 5S.
