Risk Analysis in Cup Production Process Using FMEA and TOPSIS Methods

Abstract

Production risks happening the cup production process at PT.XYZ are human factors 20%, raw material quality does not meet standards 15%, production procedure errors 40%, and packaging errors 12.5%. The purpose this study determine the priority potential risks and determine proposed improvements reduce risks in the cup production process at PT. XYZ. Research combines FMEA and TOPSIS methods. FMEA is used find highest risk. While TOPSIS is used determine the order of the most priority improvement recommendations. Based on the identification results, there are 22 risks. The highest RPN value is the temperature setting that exceeds the standard range. 5 whys are used to find the root cause. Based problems, sequence improvement recommendations is arranged based on priority using the TOPSIS method. The best alternative solution for production process risks is to provide training to operators to ensure that operators understand the standard temperature settings with an RPI of 0.680.
