PT. XYZ is a company engaged in recycled paper production, known for experiencing a higher defect rate in Quality 1 products that exceeds the company's tolerance limit. This study aims to identify the factors causing defects in recycled paper production and identify the root causes. Proposed improvements to reduce defects are suggested. The research employs FMEA and FTA methodologies. The most significant paper defect is in GSM, with root causes including insufficient GSM measurement training, inadequate supervision during material weighing, variations in raw material quality, inconsistent recycled paper materials, inadequate supplier evaluations, discrepancies in material specifications from suppliers, lack of humidity control oversight, absence of humidity control equipment, and inadequate workspace cleanliness. Proposed improvements include regular training, periodic weighing process checks, raw material quality inspections, optimization of recycled material processing, supplier selection refinement, and enhanced humidity and workspace cleanliness monitoring.
