Electronic Medical Records (EMR) are expected to improve the accuracy, timeliness, security, and accessibility of medical record information. This study aimed to describe EMR implementation and medical record service quality and analyze their association at UPT Puskesmas Galis Pamekasan. A quantitative analytic-descriptive study with a cross-sectional design was conducted in May 2026. The sample consisted of 72 active EMR users selected using total sampling. Data were collected using Likert-scale questionnaires based on the HOT-Fit dimensions and medical record service quality indicators. Data were analyzed using descriptive statistics and Spearman Rho correlation. The mean EMR implementation score was 105.75 of 135 and categorized as good, while medical record service quality scored 68.90 of 80 and was categorized as very good. Spearman Rho showed a positive and statistically significant association between EMR implementation and medical record service quality (ρ = 0.336; p = 0.004).
