The purpose of this research is to develop a web-based Medical Check Up information system using the CodeIgniter 4 framework and apply the waterfall development method. Rsu Al-Islam Hm Mawardi Krian Sidoarjo is currently still using the manual method in administering medical check-ups which poses a risk in data processing between hospital units. Therefore, this research was conducted to answer these problems. The waterfall method is used for the stages of system development which includes requirements analysis, design, implementation, testing and maintenance. Implementation uses the CodeIgniter 4 framework which provides features for web application development. The results of this study are in accordance with the needs of the hospital which includes patient registration, recording of examination results that have been tested using the black box method. It is expected that this information system can improve the efficiency of the inspection process, and improve the overall quality of health services.
