Abstract

CV. Bentonite Clay experienced various problems, including a high defect rate reaching an average of 4% per month and exceeding the company standard of 2%. The dominant types of defects include cracks, chips, breaks, and uneven surfaces with a total of 8,194 units of defects from 220,907 production units during January–March 2025. The purpose of this study was to determine the highest defects, root causes and provide recommendations for improvement. To conduct this study, the six sigma method was used to identify defect characteristics, calculate DPMO values ​​and sigma levels, and analyze the root causes of problems and kaizen to formulate continuous improvement proposals. The results showed that the highest cause was the chip defect caused by the process of releasing from the mold that was not careful, the quality of the mold was worn, or impacts during the process of moving and arranging paving blocks after production.
