Failure Mode And Effects Analysis dan Fault Tree Analysis untuk Analisis Potensi Risiko Bordir

The background of this study is the high level of product rejection in the embroidery production process, which exceeds the maximum standard by 2%. Based on data for three months, namely October to December 2025, the figure exceeds 2%. The purpose of this study is to identify the highest type of rejection, determine the highest Risk Priority Number (RPN) value, analyze the root cause of failure, and provide recommendations to repair. The method used is the integration of Failure Modes and Effects Analysis (FMEA) and Fault Tree Analysis (FTA). The FMEA method is a method to measure the level of risk, and FTA is used to identify the root cause of the problem. The results of the study show that the highest rejection is the remaining excess thread. Meanwhile, the highest RPN value of 240 occurs at the product design stage, with a color failure mode that does not match.
