PT XYZ, a powder-based food manufacturing company, is facing quality issues with 1,056 defective units out of 16,570 total production units, resulting in a defect rate of 9%, exceeding the company’s tolerance of 5%. This study aims to identify the dominant defect types, analyze root causes, determine the six sigma level, and propose improvement recommendations. The method used is Lean Six Sigma with the DMAIC approach. The results indicate that the dominant defect is hold lumpy with 305 units, caused by uncontrolled moisture content and suboptimal drying processes. The DPMO value of 12,745.93 and a sigma level of 3.7 indicate that the process performance is not optimal. In addition, defects are identified as the largest waste in the production process. Improvement efforts focus on process control, machine maintenance, work standardization, raw material control, and operator training to improve quality and production efficiency.
