CV. Karya Mumpuni is a company engaged in the production of coconut shell charcoal. This company utilizes coconut shell waste as the main raw material which is processed through a carbonization process to produce quality charcoal. The purpose of this study is to determine the highest risk value, to determine the root cause of the highest risk, and provide recommendations for improvement. The method used in this study is quantitative by integrating the DFMEA method to identify design or system failures. Fault Tree Analysis traces the root cause of the problem systematically. Based on the analysis results, it is known that the highest risk level The results of this study are that it can be seen that the highest RPN value is in the coconut shell selection process with a potential mode of failure of high-moisture coconut shells with a value of 288.
