Abstract

This research examines insulation resistance failures in newly manufactured power transformers at PT XYZ, with emphasis on the Applied Voltage Test as a key method for detecting defects. Unlike most studies that focus on aged transformers, it reveals that failures in new units may stem from production-related issues, such as insulation paper degradation caused by temperature and humidity during manufacturing. The study employed direct observation, structured interviews with testing technicians, and a comparative analysis of two transformer units. Results indicated that Unit 2 failed the test due to insulation paper damage, later confirmed through physical inspection. Findings highlight the Applied Voltage Test’s role in early detection and demonstrate how material quality and production processes directly influence transformer reliability. Additionally, the Polarization Index is identified as a valuable indicator of insulation degradation. These insights contribute to improving quality control and minimizing failure risks in transformer production and power distribution.
