Analysis of Slit-Skin Smear Sampling Effectiveness at the Right Earlobe, Left Earlobe, and Skin Lesion on Bacteriological and Morphological Index Values in Leprosy

Leprosy is a chronic bacterial infection caused by Mycobacterium leprae. Diagnosis includes detection of Acid-Fast Bacilli (AFB) by Slit-Skin Smear (SSS), yielding Bacteriological Index (BI) and Morphological Index (MI). This study compared BI and MI from three sites: right earlobe, left earlobe, and skin lesions. A laboratory experimental cross-sectional design was used, involving 30 paucibacillary and multibacillary leprosy patients. Ziehl–Neelsen staining was performed. Data were analyzed using Friedman and Wilcoxon Signed-Rank tests. Mean BI was significantly higher at right earlobe (1.93 ± 1.741) and left earlobe (1.80 ± 1.669) than at skin lesions (0.93 ± 0.691). Friedman test showed significant BI differences across sites (p = 0.000); skin lesion BI was lower than right earlobe (p = 0.001) and left earlobe (p = 0.002). MI showed no difference (p = 0.439). In conclusion, earlobes are the most effective and representative SSS sites for estimating M. leprae bacterial load.
