Abstract

Pneumonia remains a leading cause of death among infants and children under five, characterized by systemic inflammation reflected in changes in neutrophil and lymphocyte counts. This study aimed to determine the relationship between Absolute Lymphocyte Count (ALC) and Neutrophil-to-Lymphocyte Ratio (NLR) in pneumonia patients aged 1–24 months. An observational analytic study with a cross-sectional design was conducted using secondary data from the medical records of 110 pneumonia patients treated at Haji Regional General Hospital (RSUD) Surabaya from January to December 2025, selected through total sampling. Data were analyzed using the Spearman correlation test. The mean ALC was 5,153.34 cells/μL and the mean NLR was 1.74. The Kolmogorov-Smirnov test showed that ALC and NLR were not normally distributed (p<0.05). Spearman analysis revealed a significant negative correlation between ALC and NLR (r=-0.450; p<0.001). Lower ALC values corresponded to higher NLR values, reflecting immune dysregulation and systemic inflammation in pneumonia patients.
