Abstract. Patient registration at Klinik Mitra 05 Bojonegoro still relies on three separate, unintegrated systems (MyKlinik, BPJS PCare, and private insurance platforms), forcing front-desk staff to input the same patient data repeatedly and slowing down service. This study aimed to design a website-based patient registration user interface using the Participatory Design method, involving three front-office informants as active co-designers throughout the needs-analysis, prototyping, and usability-evaluation stages. Data were collected through observation, in-depth interviews, and iterative co-design sessions, then analyzed using the Miles and Huberman interactive model. The resulting high-fidelity prototype, built in Canva, introduces an insurance-bridging shortcut and a quick patient-history access button on the dashboard. The usability evaluation indicated that the design met the criteria of ease of use, efficiency, and user satisfaction, resulting in a validated interface blueprint ready for further system development.
