Coding accuracy is a critical aspect of maintaining the quality of medical records and health reporting. Based on 25 medical records of pre-marital patients, coding inaccuracies remain very low. This study aims to analyze the factors causing coding accuracy in Catin using Root Cause Analysis (RCA). This study employs a descriptive approach with data collection techniques involving RME observation and interviews. The results show that the coding accuracy rate remains very low, with the highest inaccuracy found in code Z02.8 (97%). Based on the RCA results using a fishbone diagram, the 5M factors identified were: insufficient understanding of prenatal care coding, limitations of the coders, miscommunication, limited training budget for coding, lack of discipline among primary care providers (DPJP), limited features in the RME system, and the absence of standard operating procedures (SOPs) for pre-marital patients coding.
