PT XYZ is a company engaged in the motorcycle spare parts business, located in East Java. The average product defect rate from June 2024 to November 2024 reached 10.9%, exceeding the company’s expected monthly tolerance limit (5%). The objectives of this study are to determine the highest RPN value, identify the causes of product failure, and provide recommendations for improvement. The methods used in this research is Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA). The results of this study indicate that rust on the backing plate is the failure mode with the highest RPN 140. The causes of this failure include human error, suboptimal machine parameters, incomplete quality double-checking, unstable raw material quality, and humid storage conditions. The recommended improvements include the need to enhance quality standards and SOPs across all aspects, ranging from inspection checklists and material quality to humidity control in storage areas.
