This study is driven by discrepancies between inspection reports and actual field conditions at PT XYZ, which may impact the quality of the Certificate of Operational Feasibility (SLO) for low-voltage electrical installations (IPTL TR). The research aims to assess risk levels and formulate effective mitigation strategies within the testing and inspection processes to ensure operational compliance and electrical safety. The approach adopts ISO 31000:2018 risk management integrated with a quantitative Probability Impact Matrix (PIM), covering stages from risk identification to control. The findings indicate several risks ranging from high to very high levels, with the most critical risk being contact with live electrical conductors, scoring 25. Mitigation measures are implemented through the hierarchy of risk control. This structured approach is expected to minimize risks while enhancing the quality and reliability of the SLO issuance process.
