The high rate of open joint defects in the gluing process of carton box production reduces product quality and may cause financial losses for the company. Data from July–September 2025 (36 production days) show a total output of 326,850 units with 9,358 defective units, resulting in a 2.86% defect rate. This study aims to identify the most dominant defect, analyze its root causes, and propose improvement recommendations. The methods applied were Statistical Quality Control (SQC) using control charts to evaluate process stability and Root Cause Analysis (RCA) to determine the main causes. The results indicate that open joint is the highest defect, reaching 5,265 units (1.61%). The primary cause is the lack of proper documentation and standardization in the quality management system. Recommended improvements include standardizing pressing machine parameters through written SOPs, implementing daily monitoring checklists, and conducting regular operator training.
