Analysis of Contamination Risk Management in Diaper Production Integrating Work Breakdown Structure (WBS), Risk Breakdown Structure (RBS), and Probability Impact Matrix (PIM)

This study addresses rising product contamination complaints at PT. XYZ’s diaper production, which reached 12 cases between January and September 2025 and exceeded the company’s tolerance threshold. The research aims to identify root causes and assess the highest contamination risks using an integrated approach of Work Breakdown Structure (WBS), Risk Breakdown Structure (RBS), and Probability Impact Matrix (PIM). WBS and RBS were used to map potential risks across production stages, while PIM evaluated risk severity. The findings reveal that major causes of adhesive contamination include inconsistent cleaning, insufficient monitoring, improper roll distance, overly sticky roll material, and non-compliance with cleaning SOPs. The highest risk identified was adhesive buildup on the distribution roll with a very high risk score of 25. Recommended improvements include replacing roll coatings with non-stick materials, standardizing roll distance, adding blow water, and implementing periodic cleaning schedules.
